People with a learning disability often have poorer physical and mental health than other people. An annual health check can improve people’s health by spotting problems earlier.
Who can have an annual health check?
Anyone over the age of 14 with a learning disability, You do not have to be ill to have a health check – in fact, most people have their annual health check when they are feeling well.
How to get an annual health check?
We need to know you have learning disability. Please contact us and let is know so that we can add you to our learning disability register. Once we are aware you have a learning disability, we will contact you each year to attend for your annual health check.
