A long-term condition is an illness that cannot be cured. It can usually be controlled with medicines or other treatments.
Examples of long-term conditions include diabetes, arthritis, high blood pressure, epilepsy, asthma and some mental health conditions.
If you have more one than long-term condition this is called ‘multimorbidity’.
Long-term conditions can affect your job, family and finances.
If you have a long-term condition, we can help you understand and manage your own care. This may include drawing up a care plan to help you manage your condition on a day-to-day basis and recognise symptoms that you should tell us about. A care plan may also include creating an 'information prescription'.
