The purpose of the palliative and supportive care team is to standardise end of life care for patients in their final year of life and their carers registered at Trent View Medical Practice.

• To provide a service within primary care which is underpinned by the gold standards framework (GSF).
• To provide top quality care that is more proactive, personalized to patients’ needs and wishes, with any condition, in any setting, irrespective of disease or in the absence of diagnosis.

If you, or a person you are the carer for, are being looked after by our palliative care team, you will be provided with a telephone number which gives you priority access to a member of the palliative care team. Please do not pass on that telephone number to others and please do not use it except in relation to the palliative care of the person for whom it was provided.
