  1. Title of the Group

Trent View Medical Practice Patient Participation Group (PPG)

  1. Aim of the PPG

To ensure patients and carers are involved in decisions about the range, shape and quality of services provided by their practice

  1.  Objectives of the PPG
    1. Promote the proactive and innovative involvement of patients and carers
    2. Be a critical friend of the practice
    3. Be representative to reflect the demographics of the registered population
    4. Advise the practice on the patient perspective and provide insight into the responsiveness and quality of services
    5. Encourage communities’ health
    6. Carry out research into the views of those who use the practice
    7. Support the practice to organise health promotion events
    8. Together with the Practice support on-going communication with the patient population

 

  1. Membership
    1. Open to all patients registered with Trent View Medical Practice. Membership will endeavour to reflect the patient profile and be widely representative and inclusive of different genders, ethnicities, ages and abilities as required in the GP contract, and will be a maximum of 15 people
    2. If a member does not attend three meetings in succession, the PPG will contact the member to enquire if they are still interested in attending
    3. To broaden the opportunity for membership/involvement, the PPG may consider it appropriate to also establish a Virtual Patient Participation Group (VPPG)

 

  1. Confidentiality
    1. TVMP has a responsibility to protect patient and patient related confidentiality. To maintain such confidentiality, and confidentiality related to the business, PPG members (except VPPG members [see item 7 below]) are required to agree to comply with the terms of the TVMP PPG confidentiality agreement and to sign a declaration in accordance with that policy.

 

  1. PPG Structure
    1. The PPG Chair and Secretary Officers will be formally appointed annually, at the first meeting after 1st April each year, for a term of not exceeding 3 years
    2. As an alternative to an elected Secretary, if mutually agreed, the practice may offer to provide secretarial/administrative services
    3. The Constitution and Terms of Reference to be reviewed every 12 months .
    4. Other attendees may be invited to support agenda items and discussions and/or other specific roles

 

  1. On Line
    1. A VPPG E-mail community may be developed to enhance communication in accordance with the aim and objectives of the PPG
    2. Open to all patients registered with Trent View Medical Practice

 

  1. Frequency of Meetings
    1. Every two months – no less than five times per year
    2. Dates to be agreed and circulated at least two months in advance
    3. Start time of meetings and venue to be agreed and varied to ensure maximum opportunity for attendance
    4. Meet virtually or at an appropriate office/venue
    5. Informal sub groups may be established as necessary
    6. Agenda and minutes to be available on the Practice Noticeboard and Website
    7. Agenda circulated at least three working days before the meeting
    8. Minutes to be circulated seven working days after the meeting
    9. Suggested regular Agenda items
      1. Practice / Pharmacy / Research Update
      2. KPI Figures
      3. Future Plans
      4. Feedback from members who attend external meetings relevant to the work of the PPG
      5. Correspondence
      6. A.O.B
      7. Meeting End Time – Meeting duration of ninety minutes unless agreed otherwise by the members attending

 

  1. Ground Rules
    1. Work together and support each other to meet the aim and objectives of the PPG
    2. Advocate open and honest communication and challenge
    3. Flexible, listen, ask for help and support each other
    4. Demonstrate commitment to delivering results, as a group
    5. Respect and listen constructively to others views
    6. Maintain focus
    7. Confidentiality to be respected
    8. Not to use the PPG as a forum for personal agendas or complaints. These should be taken forward through other appropriate channels
