Why do we use health questionnaires?
Patient questionnaires help us to collect simple, structured responses about your health and wellbeing. These electronic questionnaires are called "Floreys"*. Using Floreys helps us gather your health information quickly and easily, manage long-term conditions such as asthma, diabetes and mental health, and automatically add your replies and SNOMED codes to your patient record.
*Floreys were named by a company called Accurx after Sir Howard Walter Florey, a pharmacologist and pathologist who developed penicillin alongside Sir Ernst Chain and Sir Alexander Fleming.
When might I receive a health questionnaire?
There are many reasons why we make ask you to complete a health questionnaire e.g. help assess an acute condition, review an existing condition, or collect general health information.
How will I receive a questionnaire?
Floreys work by allowing the practice to send you a TEXT which contains a unique link to a pre-made questionnaire. You can then fill it out on your phone (or any compatible device with the internet) the response then comes back to us. We then save the responses straight to your health record.
If we need information from you and you don't have a mobile number registered with us, we will contact you by alternative means.
